General indicators that support eligibility
Hospice eligibility rests on clinical trajectory, not diagnosis alone. Findings that commonly support certification include:
- Progressive decline in functional status (PPS/KPS trending down)
- Unintentional weight loss or declining nutritional status
- Multiple hospitalizations or ED visits in the past 6 months
- Symptoms refractory to disease-directed treatment
- Patient/family electing comfort-focused goals of care
- Disease-specific decline despite optimal management — cardiac, pulmonary, renal, hepatic, neurologic, or oncologic
